Historical source · Original report
The 2001 investigation into Loughborough family doctor Peter Green
The original report about warnings raised by patients and staff, what happened to those warnings, and what could have been done to protect patients.
Why this report matters
CHI was an independent organisation that inspected and investigated National Health Service (NHS) care. It was replaced in 2004. Its report found that repeated warnings about Peter Green, a general practitioner (GP or family doctor), were not properly brought together. Complaints were not always taken seriously, and organisations did not have a clear picture of the risk. It recommended changes locally and nationally; see printed pages 52–58.
What the source does—and does not—establish
The report shows that listening to patients, keeping records, sharing concerns responsibly and supporting people were important issues in 2001. It does not prove that later changes failed or tell us how common this harm is today.
